Community Nursery Nurse

About this role

*To participate in the delivery of core contacts of the Healthy Child Programme to children who are part of the Universal and Targeted caseloads both in the home and in community settings. * To undertake short- and medium-term interventions with Targeted families who have been assessed as needing help with healthy weight activities, child development and/or parenting interventions. This may be by home visits or groups or interventions in an education or local authority setting. * Through supervision with line manager in 1 to 1s and within supervision groups the CHN will demonstrate competency in clinical decision making to assess and develop appropriate plans of care under the Universal and Targeted levels of the healthy child programme. * To communicate effectively, providing and receiving sensitive and complex or contentious information where advanced inter-personal skills are required. * Build effective relationships with other agencies and disciplines, and work in partnership to promote health and wellbeing of communities, families, and individuals, and to act as an advocate where appropriate. *To follow local and national policies and procedures with regards to safeguarding children. To use skills to analyse information to identity and review families causing concern and escalate to the Health Visitor to agree a plan. *Participate in the attainment of national and local public health targets and optimise opportunities for health promotion through education and practice in a community setting. *To promote vaccination programmes and other public health activities as required depending on the needs of the city and service. *To engage in service developments within scope and area of practice. *Responsibility for self and colleagues to follow Infection Control procedures. *Participate in safeguarding supervision, clinical supervision, peer support and reflective practice to ensure safe and effective delivery of the healthy child programme. *To work flexibly within the Trust to manage demands placed upon the service. *To comply with NMC Professional Code of Conduct and other related document, e.g. administration of medicines, confidentiality, and Scope of Professional Practice. *To work positively with colleagues to maintain clinically effective relationships. *Communicate effectively with children, young people, families, and colleagues always in a manner consistent with the knowledge and skills level of a Community Health Nurse. *At times the post holder may have to share information without family consent if the information is in best interests of the child and on occasions this may lead to challenges from families. *The post holder will be required to facilitate group sessions such as early parenting groups or infant feeding groups as identified locally. *Act always in a manner which respects privacy, dignity, and confidentiality. *Participate and contribute to team meetings and discussions. *Mentor and supervise new staff within the team. *Participate and contribute to teaching sessions. *Deal empathetically with sensitive information, to include discussing patients changing condition with patient and/or carer. *Experience of providing and receiving complex, sensitive information where there maybe barriers to understanding. *To be aware of and comply with current legislation and policies affecting practice, e.g. Health and Safety, Data Protection Act, Children's Act, Safeguarding Children. *To maintain good accurate contemporaneous records on SystmOne and in a timely manner. *Maintain open honest communication with the parents and children and complete the information in the parent held record book. *Record supervision, line management and annual appraisal. Be personally accountable for actions and omissions in your practice and can justify decisions taken. Ability to prioritise workload and work autonomously, instruct and direct others; work is managed, not supervised. Works within clear code of practice. Actively participate in service improvement activity to improve patient care and patient outcomes (privacy, dignity and duty of candour). *Organises, plans, and prioritises work according to service needs. *Recognises own professional boundaries and develops awareness of roles and areas of expertise of other health professionals and agencies. *Undertakes training relevant to role and attends professional meetings as required. *Develops effective working relationships with clients, colleagues, and other agencies, reporting any problem situations or workload constraints as appropriate. *Maintains confidentiality of information in line with the Data Protection Act 1998. *Assists SCPHN/Nursery Nurse with administrative work, data collection and audit activities as required to help ensure effective service provision. *Work in line with Baby Friendly Initiative (BFI). *Demonstrate knowledge of the scope of practice of the Community Health Nurse role within different care settings. *Identify personal development needs that ensure acquisition of knowledge, skills, and competency to undertake the care and treatment responsibilities assigned. *Carry out Continuing Professional Development by seeking to develop and improve practical and theoretical knowledge, competence, and skills. *Maintaining Nursing & Midwifery Council registration by adhering to all requirements regarding Standards, Proficiencies, and the revalidation process. *Develop skills in providing instruction, learning opportunities, work experience and training staff and students from other units/disciplines/schools/colleges and training schemes as required for the role. *Is proactive in seeking opportunities to develop own knowledge and skills. *To participate in self-appraisal and performance reviews. *Completes personal objectives and required competencies within agreed timeframes. *Seeks support / guidance in timely manner if any difficulties are encountered. *Works in partnership with manager to develop and deliver on Specific, Measurable, Achievable, Relevant and Time-bound (SMART) objectives at annual appraisal and personal development planning meeting. *Takes responsibility for organising and attending statutory / mandatory updates in accordance with organisational requirements. *Acts as an excellent role model by upholding and implementing good practice in the workplace. *Recognises and either directly challenges or seeks support to challenge any poor practice observed. *To apply acquired skills and knowledge of practice relevant to post and clinical area. *To review and reflect on own practice through effective use of professional and clinical supervision in line with Hampshire and Isle of Wight Healthcare NHS Foundation Trust policies and procedures. *To work within the professional, local and national guidance applicable to the clinical role being undertaken. *Staff are expected to register for the DBS Update Service. *To assist with staff induction, formal assessment, and on-going development. *Provides and receives complex, sensitive information to patients/relatives and colleagues and with empathy, persuasion and reassurance and ensure information is clearly understood, where there maybe barriers to understanding. *Actively listens, drawing out information and checks understanding. *Familiarity with the requirements of the Mental Capacity Act and maintains MCA mandatory training.
