We are seeking a dedicated and proactive Physical Health Nurse to join our valued team within the M&S MHT+. We are committed to providing exceptional, integrated physical and mental health care to individuals across Medway and Swale. This role offers the opportunity to lead and mentor a team, ensuring the delivery of high-quality physical health services to our service users within the community mental health setting.

About the Role

As a Physical Health Nurse , you will

  • Coordinate and make recommendations to the care plan process: Ensuring comprehensive and effective care plans are developed and implemented, aligning with Trust standards and the specific needs of the CMHT.
  • Mentor, supervise, and line manage staff: Providing leadership, guidance, and support to your team, fostering a positive and productive work environment within the Trust's framework.
  • Assess, act on, and report any safeguarding concerns: Ensuring the safety and wellbeing of our service users by adhering to the Trust's safeguarding policies and procedures.
  • To coordinate and make recommendations to the care plan process
  • To mentor, supervise and line manage staff.
  • To assess, act on and report any safeguarding concerns.
  • Promoting a healthy, safe and secure environment by adhering to Trust statutory health and safety requirements and maintaining confidentiality of information at all times in compliance with Trust policies, procedures, protocols and guidance.
  • Acting as a role model and supporting new team members through demonstration and explanation of tasks and techniques.
  • Facilitating, supporting and assessing staff undergoing personal and/or professional development.
  • Undertake and maintain all essential training to role.
  • Working confidently in a variety of environments according to changing service needs which present different challenges whilst continuing to act in a professional manner.
  • Being flexible and responsive to competing demands to ensure safe delivery of services.

  • The role of the Band 6 Physical Health Nurse is to facilitate clinics within the team, undertake annual physical health checks and monitoring of patients on Lithium and Antipsychotics as per the NICE Guidelines.
  • You will complete ECG's and take bloods (training will be provided).
  • The post holder will assist with the trusts view to improve the physical healthcare of all patients.
  • The postholder role will monitor patients mental and physical health, coordinating clinics and liaise closely with patients, carers, GPs, Consultant Psychiatrists and others to ensure the wellbeing of patients and to promote the general understanding of medication and monitoring issues.
  • Provide health promotion advice to patients and their carers. Participate in smoking cessation initiatives, providing support to patients and staff.
  • Relay guidance and audit on the safe use of medical devices, monitoring equipment and teaching. Support members of the Multi-Disciplinary Team on physical healthcare and infection prevention initiatives.

The post holder must hold a full, valid, UK driving licence and have access to a car/vehicle to use for business purposes (unless you have a disability as defined by the Equality Act 2010).
