Applications are invited to apply for an exciting opportunity for a 8 session 6-month fixed term locum Consultant post in Palliative Medicine.
The post is based at Meadow House Hospice (MHH). The hospice is situated in the grounds of Ealing Hospital (EHT) and is part of London Northwest University Healthcare Trust (LNWUHT). Meadow House Hospice is the base for Ealing and Hounslow Specialist Palliative Care Service which supports patients with complex needs in the context of dying throughout the London Boroughs of Ealing and Hounslow (LBE and LBH).
The clinical service at Meadow House
Hospice consists of the following
- An in-patient hospice ward at MHH with 15 individual rooms
- Two community teams – one covering LBH and one covering LBE (approximately 400 patients).
- Consultant support to the Specialist Palliative Care Team operating in Ealing Hospital
- Day Services (recently reopened)
- A 24/7 on-call consultant giving advice to healthcare professionals and patients across the London Boroughs of Ealing and Hounslow managing telephone advice to professional colleagues in local acute hospital settings.
- We host the regional lymphoedema service which treats patients with primary and secondary lymphoedema.
We are increasingly working collaboratively with the Specialist Palliative Care Service based at Northwick Park Hospital.

1. To provide consultant level specialist palliative medicine expertise to patients, palliative care teams and other colleagues across the Northwest London Health Sector
2. To play a role in improving the palliative care for patients in Meadow House Inpatient Unit, community and other care environments and improving continuity of care as patients transition between these environments.
3. To work alongside other specialist palliative care personnel to continuously improve the skills of Trust staff of all disciplines and experience in care of the dying within LNWUHT and the local communities.
4. To contribute to the palliative care team’s education and training programme for healthcare professionals on palliative care topics to develop palliative medicine and palliative care as part of the multidisciplinary palliative care team in LNWUHT. To contribute to the development of the palliative care service at LNWUHT and throughout the Northwest London health sector
5. To participate in the managerial and strategic work of the service, contributing to the constant modernisation and quality improvement of the service

The post holder will be required to
1. Work as part of the existing multidisciplinary specialist palliative care team at Meadow House Hospice.
2. Take responsibility regarding referral of patients to the service in conjunction with the multidisciplinary team.
3. Participate in patient reviews in all settings including domiciliary visits.
4. Act as a resource and support to all members of the multidisciplinary team involved in patient care.
5. Ensure that all records are kept in accordance with the Trust’s record keeping policy and in line with General Medical Council’s Good Medical Practice guidance.
6. Use electronic systems (Cerner and System1) for medical documentation.
7. Provide clinical supervision for trainees working on the inpatient unit and will liaise closely with the Educational and Clinical Supervisors of these colleagues, with the aim of assuming these responsibilities.
8. Support other members of the specialist palliative care team in managing patients with complex needs and in handling difficult consultations.
9. Take part in the appropriate referral of patients to other services, and in handover and sharing of appropriate information with other healthcare professionals.
10. Attend the weekly specialist palliative care MDTs and caseload review meetings. Be responsible for ensuring that the medical input into that meeting is available, up to date and recorded accurately.
11. Be flexible in substituting for medical colleagues (all grades) to cover both foreseen and unforeseen absences throughout the clinical team at Meadow House Hospice.
12. Provide leadership and support to the multidisciplinary team in community and inpatient settings.
