An exciting and challenging opportunity has arisen for a full-time band 7 Physiotherapist or Occupational Therapist or Nurse within the Integrated Independence Team.

We are looking for an innovative, enthusiastic and experienced Physiotherapist or Occupational Therapist or Nurse to be part of the Single Point of Access service within the Integrated Independence Team. This will be offered as fixed term (9 months) and full-time for an existing band 7 therapist or experienced band 6 therapist.

The Integrated Independence Team is an integrated acute and community Intermediate Care team. The team places the user at the heart of the care planning process, and aims to promote independence, deliver outcome focused care, and provide a seamless journey through the care system.

The team provides rapid response, intermediate care therapy and reablement services and consists of Occupational Therapists, Physiotherapists, Psychotherapists, Nurses, Speech and Language Therapists, Consultant Geriatricians, Social Workers, Independence Coordinators, Independence and Specialist Independence Assistants and Administration staff. The team will use integrated support planning to address health and social care needs across the pathway in a responsive, dynamic and flexible way.

Working Hours

37.5 per week (flexible working or compressed hours will be considered)

We are looking for a Band 7 Physiotherapist or Occupational Therapist or Nurse to provide clinical co-ordination and triaging of all referrals into IIT including referrals for the Rehab at Home service, the Community Rapid Response service and the Discharge to Assess service.

In doing this you will work as part of a strong multidisciplinary team, within an interdisciplinary working model, to facilitate hospital discharge and prevent acute admission. The role is mostly office based but there are many opportunities to provide clinical assessment/intervention to patients to support other IIT services such as Rehab at Home, Discharge to Assess or Rapid Response.

The main duties of the job are

  • Together with colleagues, lead and coordinate the IIT Single Point of Access service, screening all referrals and allocating cases to appropriate care pathways
  • Liaising with primary, secondary, health and social care providers, to case manage individuals into appropriate service or declining referrals, where necessary
  • Promote independence for service users, enabling them to live in the least restrictive environment whilst ensuring they receive the necessary care and treatment
  • In-reach to acute hospital ward as required
  • Assess clients referred, when required, to decide on appropriateness for the service
