We are offering an exciting opportunityfor an experienced, innovative Paediatric Nurse to join the Children's Community Nursing Service in Newham as the Wellchild Childrens Complex Needs Nurse. This places you at the heart of compassionate, community‑based care, supporting children with life‑limiting conditions and their families at the most critical moments of their journey.
The Complex Care Nurse Specialist will be responsible for leading the pathway for children with complex care needs. You will work alongside the Core Nursing Pathway Lead and Palliative and End of Life Pathway Lead and sits within a wider multi-disciplinary service including Nursing (Specialist School Nursing, Asthma, Epilepsy, Continuing care) and Community paediatric dietitians. The Pathway Lead will also be expected to have an MDT approach to patient care with the psychology and play therapy services .
You will be responsible for the over site of children with complex health needs in the community setting, this may include holding a case load of your own as well as supporting and supervising the band 6 children's community nursing to deliver nursing care to children and their families.

To provide expert nursing advice and support to children with long term complex health needs and their families across the borough of Newham.
To liaise with community and hospital services partner organisations and local authority services to provide a comprehensive high quality seamless discharge from hospital to home.
To lead the complex childrens nursing pathway in Newham, to ensure delivery of high quality, responsive nursing care to children and their families and to be the named lead healthcare professional for this pathway.
This role works alongside the nursing leads for the palliative and end of life pathway and core nursing service pathway, together delivering co-ordinated care for the children and young people seen by the community nursing services. The pathway leads will work together to ensure children receive seamless, expert and timely care to meet their health and holistic needs.
