About this role

Clinical Care Provide compassionate, person-centred nursing care for adults with complex physical, mental health and social care needs. Undertake delegated nursing assessments within scope of practice, contributing to the development, implementation and evaluation of individualised care plans. Deliver planned nursing interventions in accordance with evidence-based practice and local clinical guidelines. Recognise changes in patients' physical, psychological and cognitive condition and escalate concerns appropriately. Support patients and families to maximise independence, self-management and wellbeing. Maintain professional accountability in accordance with NMC standards. Long-Term Condition Management Support proactive management of diabetes, hypertension, COPD, asthma, cardiovascular disease, chronic kidney disease and frailty. Undertake delegated reviews within agreed competency frameworks. Support health promotion, lifestyle advice and self-management education. Complete diabetic foot assessments Contribute to QOF and enhanced service delivery. Frailty and Community Outreach Undertake planned home visits for housebound patients. Support comprehensive frailty assessments alongside registered clinicians. Recognise deterioration using NEWS2 where appropriate. Support admission avoidance and coordinated care. Support patients approaching end of life alongside the multidisciplinary team. Clinical Skills Venepuncture Physiological observations Height, weight and BMI Urinalysis and point-of-care testing Wound assessment and simple wound care (within competency) Pressure area assessment Catheter care and sampling Assessment and Risk Identification Identify and escalate risks relating to deterioration, falls, pressure ulcers, malnutrition, dehydration, infection, safeguarding, self-neglect, mental health deterioration, carer strain and social isolation. Care Coordination Work collaboratively with GPs, Practice Nurses, Community Nurses, Allied Health Professionals and Social Care. Participate in MDT meetings. Support referrals and continuity of care. Medicines Support medicines optimisation within scope of practice. Administer medicines where trained, competent and authorised under local policy. Monitor therapeutic effects and adverse reactions. Health Promotion Promote healthy lifestyles, vaccination, falls prevention, nutrition and self-management. Professional Responsibilities Maintain NMC registration and CPD. Participate in appraisal, supervision and reflective practice. Support learners and new staff where appropriate. Maintain confidentiality and information governance. Clinical Systems Use EMIS and or SystmOne to document care accurately, code activity and contribute to audit and quality improvement. Lone Working Manage a community caseload safely. Follow lone worker policies. Work autonomously within professional competence.
