Senior Rehabilitation Assistant

The role is based in Responsive Services which comprises of the Intermediate Care Team, Virtual Ward, Urgent Community Response and Referral Hub. The Senior Rehabilitation Assistant will be an integral part of the Responsive Services team, supporting the rehabilitation assistants and allocating work as well as having their own delegated caseload. The post holder will complete basic assessments of patients in their home and provide equipment under the guidance of a qualified clinician. They will carry out rehabilitation programmes with patients to increase independence to enable patients to remain in their own home.
