What you will do
•
Assessment and Support Planning
Undertake strengths-based and needs-led assessments, developing personalised care and support plans.
•
Hospital Admission Prevention
Support adults and carers through interventions designed to reduce unnecessary hospital admissions and promote community-based care.
•
Safeguarding
Conduct safeguarding enquiries in accordance with the Care Act 2014, ensuring individuals receive appropriate protection and support.
•
Multi-Agency Working
Collaborate with health professionals, community teams, carers, and partner organisations to deliver coordinated services.
•
Communication
Effectively communicate complex information to individuals, carers, and professionals using a range of methods and approaches.
•
Meeting Coordination
Organise, chair, and contribute to multidisciplinary meetings, ensuring accurate records and action plans are maintained.
•
Advocacy
Promote the rights, independence, and wellbeing of adults, challenging discrimination and barriers linked to age, disability, or mental health needs.

Community Engagement
Participate in community initiatives and forums to improve service delivery and strengthen local support networks.
•
Record Keeping
Maintain accurate, timely, and compliant case records in line with statutory and organisational requirements.
Qualifications / Requirements
•
Social Work Qualification
Degree in Social Work or equivalent recognised qualification.
•
Registration
Current Social Work England registration.
•
Experience
Previous experience within Adult Social Care, community services, hospital discharge, or integrated health and social care settings.
•
Legislative Knowledge
Strong understanding of the Care Act 2014, Mental Capacity Act 2005, safeguarding procedures, and related legislation.
•
Assessment Skills
Experience undertaking strengths-based assessments, care planning, and risk management.
•
Communication Skills
Excellent written, verbal, and interpersonal communication abilities.
•
Partnership Working
Ability to work effectively within multidisciplinary and multi-agency environments.
•
Organisational Skills
Strong case management and prioritisation skills within a fast-paced service.
