About the role
As an Outreach Worker, you'll be a vital first point of contact for people who are often excluded from traditional services, including rough sleepers, people with complex needs and those experiencing physical health challenges.
No two days are the same. You'll build trusted relationships, reduce harm and help people take their first steps towards recovery by:
- Engaging with people in the community who may not currently be accessing support.
- Providing harm reduction advice and interventions, including overdose prevention and blood-borne virus awareness.
- Completing holistic assessments and developing personalised care plans.
- Supporting people into treatment and ongoing recovery.
- Advocating on behalf of service users with housing, healthcare, social care, benefits and other local services.
- Working flexibly across the community to meet the needs of those who need us most.
