The services we provide are commissioned by NHS Surrey and Sussex Integrated Care Board (ICB), and we do not provide private healthcare. We work collaboratively with the ICB, General Practice and Community Services to develop and deliver new pathways of care and to ensure efficient and effective use of NHS resources. For more information about Dorking Healthcare, please visit: https://dhcclinical.co.uk/ JOB SUMMARY The main aim for this job role is to provide high-quality patient-centred care to all patients attending the Dermatology service. The delivery of onsite clinical sessions within a community-based environment, where this could include general adult and child Dermatology clinics, Isotretinoin initiation clinics and minor surgery. The person will work with the existing clinicians to expand service provision for specialist dermatology referrals. The post holder will maintain an active clinical role as a member of the dermatology clinician body, delivering excellent dermatology care for DHC patients. Supervision of GP trainees, GPs on an extended role dermatology programme and medical students is required and will be job planned accordingly. The post holder will be expected to show drive and initiative towards delivering excellent patient care. The successful candidate will possess a recognised Medical degree, MRCP or FRCP and be on the specialist register for Dermatology with the GMC. It is a condition of appointment that staff are satisfactorily health cleared prior to commencing work. You will be part of a team currently comprised of 3 consultants, 2 GPwERs and 5 GPwSIs. You will work closely with clinical, nursing and administrative teams, in particular with the Dermatology Services Coordinator, who is responsible for supporting the management and clinical teams and the clinical lead. The Dermatology Services Coordinator is responsible for looking after the Dermatology Pathway from start to finish, including processing inbound referrals, booking appointments and scheduling minor operations, managing the number of clinics available depending on patient demand, chasing pathology results and managing waitlists. There is no on-call requirement with this post. DUTIES AND RESPONSIBILITIES OF THE POST The post holder must adhere to standards as outlined by the GMC for Good Medical Practice. Ensure prompt attendance at agreed direct clinical care programmed activities. To provide a full range of dermatological services to NHS patients referred into DHCs Dermatology Outpatients service. Offer procedures such as cryotherapy, injections and surgical intervention as appropriate. Arrange investigation, offer management plans for GP implementation (returning patients to GP care) or to offer follow-up (either face to face or as a telephone consultation) or onward referral to secondary care as needed Ensure patients are involved in decisions about their care and to respond to their views. Maintain adequate and timely records of consultations, investigations, prescribing and procedures using the clinical system (currently SystmOne) and its templates &/or a separate database where appropriate. Complete computerised outcome letters to the referring clinician/ patient/ carer and liaise with the administration team to ensure that letters reach them Ensure timely review of results with communication of the outcome to the patient or their carer and/or referring clinician as appropriate. Practise within national USC/ NICE/ IOG/ BAD/PCDS/ local dermatology guidelines. Prescribe in line with local dermatology formularies. To maintain confidentiality whilst communicating patient information, in such a way that preserves the dignity and privacy of the patient and the patients family and carers. Handle data/ images/ patient information in a GDPR compliant manner e.g. ensuring the use of NHS.net email addresses for the transfer of patient-specific information. To maintain clinical competence and exercise awareness of professional responsibilities by reading current relevant literature and undertaking opportunities for personal and professional development in accordance with the requirements of the GMC. To be personally accountable for professional and ethical actions and practices within a legal and ethical framework that adheres to the GMC's requirements and local policies and procedures. Attend or participate in regular brief administrative and clinical meetings with colleagues. Participate in research programmes if appropriate. Endeavour to ensure that services are responsive to the needs of people affected by skin conditions and will assist in patient surveys as requested. To work with colleagues, both medical and nursing, to improve and advance the service. Lead the development of dermatology services through the implementation of new diagnostic and treatment modalities, strengthening and complementing existing service provision. Lead and participate in clinical audit and quality improvement activities within dermatology, ensuring practice is aligned with current evidence, national guidance, and best standards of patient care. To have responsibility for ensuring active participation in continuing professional development (CPD) as well as completing mandatory training and complying with relevant regulations. Demonstrate, model and encourage DHC values at all times to all patients and staff. Support training and development needs of GPs with an extended specialist role (GPwSI and GPwER). Supporting training of other clinical staff working as part of the dermatology team. Attend appropriate MDT meetings as part of local NHS cancer network services. INDIVIDUAL RESPONSIBILITIES Ensure that your own and other relevant clinicians mandatory training and other applicable training records and accreditation are kept updated. WORK SETTING AND REVIEW 1.
Role in clinic
Three types of clinics A. Normal - No teaching 15 min New 10 min FU B. Standard Teaching - with 15 min teaching slots per GPwER / GPwSI C. Teaching - limited patient slots for consultant - sole purpose supervision of less experienced GPwSIs e.g. Mini Cex, CBDs Complex cases, teaching cases, clinical supervision and shadowing. Each clinic will be 4 hours in duration. It is anticipated that 3 hours will be patient-facing and 1 hour will be for administration of the clinic and other non-clinical responsibilities. 2.
Role Outside of clinic
A. Teaching and Supervision Review and approval of both DHC Accreditation of GPwSI / GPwER starting at DHC and the annual DHC Appraisal documents - this would require minimal input, but appropriate thought for development of the Drs concerned from a PDP perspective with the goal of progressing each clinician in the service to deal with more complexity and ultimately be accredited by BAD as GPwER. Initial accreditation is a one off, appraisal is annual. Familiarise oneself with the requirements of GPwER Dermatology as per BAD. B. Growth and Development of the DHC Dermatology Service Work with the Clinical lead GPwER to review and develop protocols and appropriate SOPs for various aspects of the service to ensure three objectives: 1. Safe delivery of the service by all clinicians 2. Consistency in good practice by all clinicians in the service 3. That the service delivery and SOPs are up to date and compliant with NICE guidance/BAD guidelines 4. To participate in regular clinical audit to demonstrate the effectiveness of clinical care delivered by DHC. C. Availability for ad hoc advice from primarily the Clinical Lead GPwER, but at times other Drs working in the service.
