About this role

Reviewing of commissioned care and support for people with LDA/S117 with partners in the Local Authority and provider TrustSupport discharges from Acute hospitals, Assessment and Treatment units and any other type of inpatient unit which provides care for people with learning disability, autism or both. Complete Inpatient Care and Treatment Reviews, Community Care Education, and Treatment Reviews and eight-week commissioner oversight visits, to support discharge from mental health hospital and to prevent hospital admission. Support with admission avoidance work for people with a learning disability, autism, or both, in the community, via the Dynamic Support Register. Work alongside and reporting to the Learning Disability and Autism Programme, Programme team to meet inpatient trajectory targets and other LDA related, as set by NHS England. Support Adult and Childrens Safeguarding processes, ensuring that care packages and commissioned services are safe and high quality Use specialist knowledge and/or clinical skills to ensure that coordination of care and outcomes are determined in a consistent manner to ensure effective application and interpretation of the MHA S117 and National Framework for NHS CHC and NHS FNC. Have good verbal and written communication skills to communicate with people with Learning Disability, Autism or both, their families and other professionals. The production of accurate, detailed and comprehensive reports is essential. Work closely with hospitals, community services and care providers to develop systems and processes to facilitate a smooth processes and responses to a range of stakeholders. Undertake complex case management for people with learning disability, autism or both funded by the ICB in hospital or with S117 aftercare needs in the community via packages of care and support. Jointly commissioning care packages which are in accordance with locally agreed protocols within the ICB and Local Authority. To review the holistic needs of the person with Learning Disability, Autism or both regarding the appropriateness of the placement, level of care being provided, safeguarding and entitlement to NHS Funded Health Care. Ensure accurate and open communication and co-ordination with a range of organisations and individuals. Participate in training and development to ensure continuing professional development Set up Personal Health Budgets where appropriate, monitor and review following PHB policies and processes. Coordinate and/or multi-disciplinary meetings, multi agency risk meetings where there is a person in crisis. Review Nursing Home placements for older people with mental health illness ensuring that care is high quality, value for money and meeting the needs of the person. Have a good working knowledge of Mental Capacity Act, Deprivation of liberty and Best Interests consultation. Complete LeDeR mortality reviews for people with learning disability and/or autism.
