About this role
Beckington Family Practice is seeking an experienced Clinical Pharmacist Independent Prescriber to join our friendly multidisciplinary team for 16 hours per week This is a predominantly patient-facing role, with approximately three-quarters of the role devoted to running pharmacist-led clinics and one-quarter supporting medicines safety and quality improvement. We are looking for someone who enjoys managing patients independently, building long-term relationships and developing clinical services Main responsibilities Clinical Care - approx 12 hours per week Run independent pharmacist-led clinics Manage hypertension, lipid disorders and cardiovascular risk Provide medication optimisation and long-term condition management within your competence Independently prescribe, review medicines and request/interpret appropriate investigations Provide lifestyle advice and shared decision-making Refer patients appropriately to GPs or other healthcare professionals Maintain accurate clinical records Develop additional clinics aligned with your expertise and practice needs Medicines Safety and Quality Improvement - approx 4 hours per week Review and implement MHRA Drug Safety Alerts Complete medicines safety and CQC prescribing searches Undertake prescribing audits and quality improvement projects Identify patients requiring medication monitoring Support implementation of local and national prescribing guidance Contribute to medicines governance across the practice Professional Responsibilities The successful applicant will maintain GPhC registration and CPD, work within their professional scope of practice, comply with all relevant practice policies (including confidentiality, safeguarding and information governance), participate in appraisal and mandatory training, and contribute positively to the multidisciplinary team Other duties To present a professional image of Beckington Family Practice to the patients and wider public To undertake any other delegated duties reasonably considered appropriate to the post Requirements of the post Smart, tidy appearance, wearing uniform if provided An understanding, acceptance and adherence to the need for strict confidentiality An ability to use judgement, resourcefulness, common sense and tact to respond appropriately to the needs of the post Excellent communication skills both face to face and on the telephone A flexible approach to the position in order to provide support for other team members Able to use own initiative To be logical, self-motivated, resourceful and attentive to detail Ability to ask for help and use the knowledge of the Senior Team for support Circumstances Reliable Able to be flexible if needed to work extra at other times Contraindications Being a patient with the Practice once employed Quality The post-holder will strive to maintain quality within the Practice, and will Alert other team members to issues of quality and risk Assess own performance and take accountability for own actions, either directly or under supervision Contribute to the effectiveness of the team by reflecting on own and team activities and making suggestions on ways to improve and enhance the teams performance Participate in team meetings as required Work effectively with individuals in other agencies to meet patients needs Effectively manage own time, workload and resources Confidentiality All employees of Beckington Family Practice are required to maintain and ensure the confidentiality of members of the public and members of staff in accordance with Beckington Family Practice information security and Confidentiality policies and in accordance with the Data Protection Act 1998 and Caldecott principles Collation of Information Support the Practice team in achievement of keeping medical records updated by ensuring the following Collation of patient information using Emis Web medical information system Maintaining quality and accuracy of all data input Comply with Data Protection Act and GDPRCommunication The post-holder should recognize the importance of effective communication within the team and will strive to Communicate effectively with other team members To offer a polite, friendly and efficient level of communication service to all our patients, ensuring that a friendly, caring, efficient, professional and consistent service is provided to patients at all sites To treat patients, colleagues and visitors with dignity and respect at all times To work in partnership with our patients to help them achieve better health outcomes Recognize peoples needs for alternative methods of communication and respond accordingly Maintain a professional demeanour and uphold high service standards Working Environment Carry out routine checks to ensure your work station is clean and tidy at the start and end of your shift Ensure all equipment in your work station is operational and up to date and report any issues to your line manager Ensure that confidential patient information is stored and handled according to protocol and report any concerns to your line managerPolicies & Procedures Apply Practice policies, standards and guidance as per practice shared drive and staff handbook Discuss with other members of the team how the policies, standards and guidelines will affect own work Ensure you keep abreast of policy updatesRecords Management As an employee of Beckington Family Practice, you are legally responsible for all records that you gather, create or use as part of your work within the Organisation, including patient health, financial, personal and administrative, whether paper based or on computer. Risk Management It is a standard element of the role and responsibility of all staff of Beckington Family Practice that they fulfil a proactive role towards the management of risk in all of their actions. This entails the risk assessment of all situations, the taking of appropriate actions and reporting of all incidents, near misses and hazards Health & Safety All employees of Beckington Family Practice have a statutory duty of care for their own personal safety and that of others who may be affected by their acts or omissions. Employees are required to co-operate with management to enable the practice to meet its own legal duties and to report any hazardous situations or defective equipment.
