Navigator Job Description Responsible to

Practice Manager Job Purpose The Care Navigator is the first point of contact for patients and visitors, providing a professional, efficient and compassionate service. The role involves assessing patient enquiries using care navigation principles, directing patients to the most appropriate clinician or service, and supporting the smooth running of the practice through reception and administrative duties. Key Responsibilities Welcome patients and visitors in a friendly and professional manner. Assess patient requests and signpost them to the most appropriate healthcare professional or service. Book, amend and manage appointments in line with practice protocols. Answer telephone calls, respond to patient enquiries and take accurate messages. Process repeat prescription requests and communicate test results in accordance with practice procedures. Register new and temporary patients and maintain accurate electronic patient records using EMIS Web. Scan, code and process clinical correspondence and other documentation. Maintain confidentiality and comply with GDPR, NHS Wales and practice policies. Assist patients with online services, digital access and general practice information. Maintain a safe, tidy and welcoming reception environment. Support opening and closing procedures where required. Work collaboratively with the multidisciplinary team to provide high-quality patient care. Undertake mandatory training and any other duties appropriate to the role. Key Skills Excellent communication and customer service skills. Ability to remain calm and prioritise work in a busy environment. Good IT and keyboard skills. Ability to work independently and as part of a team. Professional, empathetic and confidential approach when dealing with patients.
