What you will do

• Conduct assessments and reviews of children with disabilities and their families
• Develop and implement care plans that address the unique needs of each child
• Support families in navigating services, addressing challenges, and accessing resources
• Work collaboratively with family support workers and direct payments workers
• Maintain accurate records and prepare reports for case management
• Participate in multi-agency meetings to ensure coordinated support for families
