What you will do

• Act as a key practitioner within the Front Door Service, receiving, screening, and triaging adult social care referrals.
• Conduct assessments under the Care Act 2014, including carers assessments and proportionate reviews, to determine eligibility and support needs.
• Manage safeguarding concerns and enquiries in accordance with Section 42 of the Care Act and adhere to local safeguarding procedures.
• Offer information, advice, and signposting to adults, carers, and partner agencies to promote prevention and early intervention.

Person Specification

Qualifications

• Degree in Social Work, CQSW, DipSW, or an equivalent recognised qualification in social work.
• Evidence of ongoing professional development related to Adult Social Care practice.

Registration & Compliance

• Current registration with Social Work England (SWE).
• Proficient in understanding and applying critical legislation and guidance such as the Care Act 2014, Mental Capacity Act 2005, Human Rights Act 1998, Equality Act 2010, and Safeguarding Adults statutory guidance.
• Ability to navigate and implement local authority policies and procedures effectively.
