About this role
The learning disability health facilitation team, support people with a diagnosed learning disability to have equitable access to community or acute care. This exciting and new practitioner role will support and enhance the current acute liaison nurse role in UH Sussex sites to improve the care outcomes for eligible patients. We would consider a job share with two part time applicant's providing practitioner support to both sites.
