Detailed job description and main responsibilities To lead the delivery of the Healthy Child Programme, and ensure the 4, 5, 6 Service model is implemented in practice. Ensure services are provided within the HCP model- Your Community, Universal, Targeted and Specialist Services This will incorporate building community capacity, and embedding this into caseload, team and locality management. Responsible for the management of caseload allocation as per locality/team, and classify cases in line national stratify of Universal, Universal plus and Universal partnership plus. Work in accordance to the Locality commissioned service specification, to ensure the 5 mandated reviews are completed within required timescales, services provided are evidence based practice and the 6 High impact areas are embedded within practice. To improve the health and wellbeing of children and young people especially those from disadvantaged groups. To Identify, prioritise, develop and implement programmes of care to meet individual and local needs, taking into account cultural diversity. This will be achieved through evidence based assessments, effective care planning and implementation of a range of support packages for individual families, targeting the most vulnerable families and groups utilising best available evidence . Starting with NELFT NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop-in sessions focusing on engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex. Probationary Period This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).
