What you will do
•
Occupational Therapy Assessments
Conduct comprehensive assessments using a strengths-based and person-centred approach.
•
Rehabilitation Planning
Develop and implement goal-focused rehabilitation and outcome-based intervention plans.
•
Promoting Independence
Support individuals to maximise functional independence and safely return to their own homes.
•
Discharge Planning
Lead and coordinate discharge planning for individuals leaving Recovery Hubs.
•
Multidisciplinary Working
Collaborate closely with Physiotherapists, Nurses, Support Workers, and other healthcare professionals.
•
Intervention Reviews
Monitor progress and adapt rehabilitation plans to ensure optimal outcomes.
•
Moving and Handling Assessments
Complete moving and handling assessments and develop effective risk management strategies.
•
Equipment and Adaptations
Recommend interventions and solutions that balance safety, independence, and personal goals.
•
Record Keeping
Maintain accurate, timely, and professional records in accordance with statutory and organisational requirements.
•
Service Delivery
Deliver a high-quality Occupational Therapy service to adults, children, young people, carers, and the wider community where appropriate.
Qualifications / Requirements
•
Occupational Therapy Qualification
Degree in Occupational Therapy or equivalent recognised qualification.
•
Registration
Current HCPC registration as an Occupational Therapist.
•
Experience
Previous experience within intermediate care, rehabilitation, community services, or discharge planning is desirable.
•
Assessment Skills
Strong experience in functional assessments, rehabilitation planning, and risk management.
•
Moving and Handling Knowledge
Confidence in undertaking moving and handling assessments and implementing safe solutions.
•
Communication Skills
Excellent verbal, written, and interpersonal communication abilities.
•
Teamwork
Ability to work effectively within a multidisciplinary healthcare environment.
•
Person-Centred Practice
Commitment to promoting independence, choice, and positive outcomes for service users.
