What you will do
- Assess the needs of vulnerable adults, ensuring their views, wishes, and rights are central to decision-making.
- Develop, implement, and review person-centred care and safeguarding plans.
- Work collaboratively with healthcare professionals, local authorities, and partner agencies to deliver coordinated support.
- Undertake safeguarding enquiries and risk assessments, identifying and managing risks appropriately.
- Maintain accurate, timely, and comprehensive case records in line with statutory and organisational requirements.
- Provide advice, guidance, and support to individuals, families, and carers to promote wellbeing and independence.
Qualifications and Requirements
- Degree or equivalent qualification in Social Work.
- Current registration with Social Work England.
- Previous experience within Adult Social Care, with experience in safeguarding desirable.
- Strong knowledge of the Care Act 2014, Mental Capacity Act 2005, safeguarding legislation, and relevant statutory guidance.
- Excellent assessment, communication, and case management skills.
- Ability to work effectively as part of a multidisciplinary team.
