What you will do
•
Assessment and Planning
Complete comprehensive assessments, risk assessments, and personalised recovery plans for individuals entering the service.
•
Case Management
Manage a caseload of service users, ensuring timely access to treatment, interventions, and ongoing support.
•
Psychosocial Interventions
Deliver one-to-one support sessions and facilitate group-based interventions where required.
•
Criminal Justice Support
Assist individuals transitioning from custody into community-based treatment services.
•
Multi-Agency Working
Work closely with Probation Services, Courts, Prisons, Police, healthcare providers, and community organisations.

Advocacy
Support service users in accessing housing, healthcare, employment, education, and welfare services.
•
Harm Reduction
Promote harm reduction approaches to minimise substance-related risks and reduce reoffending.
•
Record Keeping
Maintain accurate, confidential, and compliant case records in accordance with organisational policies.
•
Outcome Monitoring
Monitor recovery progress, evaluate outcomes, and adjust support plans as appropriate.
•
Service Development
Contribute to quality improvement initiatives and support the ongoing development of services.
Qualifications / Requirements
•
Experience
Previous experience working within substance misuse, criminal justice, recovery services, or related support environments.
•
Case Management Skills
Experience managing caseloads and delivering person-centred interventions.
•
Knowledge
Understanding of substance misuse, recovery models, harm reduction approaches, and criminal justice pathways.
•
Communication Skills
Excellent interpersonal, motivational, and engagement skills.
•
Partnership Working
Ability to work effectively with multiple agencies and stakeholders.
