What you will do
- Provide occupational therapy services as a Care Coordinator, supporting service users with their individual needs.
- Develop and implement personalised care plans to promote independence and wellbeing.
- Work collaboratively with multidisciplinary teams to ensure effective and coordinated care.
- Use EPJS to maintain accurate and up-to-date patient records.
- Engage with service users and their families, providing guidance, support, and education.
- Travel within the Croydon area to deliver services effectively, requiring a valid driver's licence.
Qualifications/Requirements
- Previous experience working as an Occupational Therapist within a relevant setting.
- HCPC registration as an Occupational Therapist.
- Experience using EPJS is desirable.
- Ability to travel within the Croydon area with a valid driver's licence.
