About this role
Support patients with low to moderate mental health needs, in 1:1 or group sessions, in person or remotely To provide brief personalised support to individuals, families, and carers to ensure that they are active participants in their own healthcare and are empowered to take more control in managing their own health and well-being, live independently and improve their health outcomes through the following a) Providing interventions such as self-management, education, and peer support b) Signposting patients to the correct healthcare professional c) Supporting people to establish and attain goals d) Working with the social prescribing service to connect patients with community-based activities that support their health and well-being To coach and motivate patients through multiple sessions to identify their needs set goals and support them to implement their personalised health and care plan Develop links with local mental health services, social care and voluntary organisations to support patients health and well-being holistically Attend practice clinical and/or multi-disciplinary team meetings and report to the monthly PCN meeting as requested To liaise with the practices and, where practicable, standardise processes across the PCN To promote health coaching and social prescribing amongst the PCN and networked practices and to support colleagues to improve their skills and understanding of personalised care. To maintain accurate clinical records of all patient consultations and related work To receive referrals from triage services and other clinicians Miscellaneous Support delivery of QOF, incentive schemes, QIPP and other quality or cost effectiveness initiatives Assist with the gathering of statistics and information when required, as requested by Health Professionals and
Practice & PCN Management Safeguarding
The post holder will be expected to undertake safeguarding training (Children and Adults) appropriate to their role and adhere to policies and procedures relevant to the area they work in.

Personal/Professional Development
The post-holder will participate in any training programme implemented by the Practice/PCN as part of this employment, such training to include: Participation in an annual individual performance review, including taking responsibility for maintaining a record of own personal and/or professional development Taking responsibility for own development, learning and performance and demonstrating skills and activities to others who are undertaking similar work. You will be required to keep a record of the patients are seeing for audit purposes.
