About this role

A Vacancy at York and Scarborough Teaching Hospitals NHS Foundation Trust.

We are looking for a dynamic individual who is confident in their clinical practice and who can drive forward service transformation inline with the national agenda.

The post holder will be based within the York and Selby Community Inpatient Units and will play a key role within Community and Care of the Elderly Services. As part of our commitment to delivering integrated, neighbourhood based care, the post holder may work across a variety of community teams, supporting collaborative working and helping to provide high quality, person centred care closer to home.

The aim and main function of the community in-patient unit bed base is to provide short term rehabilitation, reablement, recovery and end of life care to the patients.

The post holder will practice as an Advanced Clinical Practitioner within the Community Inpatient Units, they will join a Team of Advanced Clinical Practitioners and Trust Grade Doctor to provide patient-centred, high-quality care.

The role will encompass the skills of assessment, examination, diagnosis and treatment within this area pf practice. Supporting admission from the Hospital and the community and timely patient discharges from the in-patient units.
