What you will do

• Facilitate Pathway 1 hospital discharges by engaging with customers on the day of discharge.
• Conduct comprehensive assessments and develop support plans to aid clients’ reablement.
• Collaborate closely with multidisciplinary teams to ensure holistic care approaches.
• Monitor and evaluate clients' progress, adjusting plans as required to ensure optimal outcomes.
• Provide guidance and educational support to clients and their families to promote health and wellbeing.
