What you will do

Assessment

Conduct comprehensive assessments of adults' needs, strengths, and support requirements.

Care Planning

Develop personalised care and support plans that promote recovery, independence, and wellbeing.

Build strong professional relationships through face-to-face engagement and ongoing intervention.

Case Management

Qualifications / Requirements

Social Work Qualification

Degree in Social Work or equivalent recognised qualification.

•

Registration

Current Social Work England registration.

Experience

Previous experience working within Adult Social Care and Mental Health Services.

Mental Health Knowledge

Strong understanding of mental health legislation, safeguarding frameworks, and person-centred practice.

Experience undertaking comprehensive assessments, care planning, and risk management.
