About this role
The Discharge Coordinator team are responsible for the management of complex discharges of patients referred to the Transfer of Care Hub. discharge Coordinators work with patients, families and members of the Multi-Disciplinary Team (MDT) to facilitate safe, appropriate and timely discharge of this patient group through assessment of care needs to enable identification of a facility that can meet these needs.

This is a rare an exciting opportunity for an experienced nurse to join our Discharge Coordinator team which is part of the Transfer of Care Hub. As a Discharge Coordinator you will be responsible for the assessment and discharge planning of patients with complex discharge needs including those that are nearing the end of their life and are in the terminal phase of their illness.
You will work as part of multi-disciplinary, multi-organisation team to ensure safe and timely discharges for this patient group. Please see the attached job description for full details of this role.
