What you will do
•
Case Management
Manage a varied caseload of young people transitioning from Children's Services to Adult Social Care.
•
Assessment
Undertake strengths-based assessments in accordance with the Care Act 2014.
•
Care Planning
Develop, implement, and review personalised care and support plans that reflect individual needs, aspirations, and outcomes.
•
Transition Planning
Work collaboratively with Children's Services, Adult Social Care teams, and partner agencies to ensure smooth and effective transition arrangements.
•
Family Support
Provide guidance and support to young people and their families regarding available services, resources, and funding options.
•
Mental Capacity Assessments
Complete Mental Capacity Assessments and Best Interest decisions in line with the Mental Capacity Act 2005.
•
Safeguarding
Identify, manage, and respond appropriately to safeguarding concerns.
•
Advocacy
Promote independence, inclusion, self-determination, and person-centred practice.
•
Multi-Agency Working
Participate in multidisciplinary meetings, transition reviews, and safeguarding processes where required.
•
Record Keeping
Maintain accurate, confidential, and timely records using electronic case management systems.
•
Compliance
Ensure all work is completed in accordance with statutory duties, council policies, and professional standards.
Qualifications / Requirements
•
Social Work Qualification
Degree in Social Work or equivalent recognised qualification.
•
Registration
Current Social Work England registration.
•
Experience
Previous experience within Adult Social Care, Children's Services, or Transitions Services.
•
Legislative Knowledge
Strong understanding of the Care Act 2014, Mental Capacity Act 2005, safeguarding frameworks, and relevant legislation.
•
Assessment Skills
Experience undertaking strengths-based assessments and person-centred care planning.
•
Communication Skills
Excellent written, verbal, and interpersonal communication abilities.
•
Partnership Working
Ability to work effectively with families, carers, and multidisciplinary professionals.
•
Organisational Skills
Strong case management and prioritisation skills within a busy service environment.
