What you will do

  • Assess patients admitted to general medicine rehabilitation and those requiring admission avoidance support.
  • Develop and implement personalised care plans to promote recovery, independence, and improved quality of life.
  • Collaborate with multidisciplinary teams to deliver effective and holistic patient care.
  • Monitor and document patient progress, adapting interventions where required.
  • Provide advice, guidance, and education to patients and their families regarding treatment plans and coping strategies.

Qualifications/Requirements

  • HCPC registration is essential.
  • A full driving licence is required as travel to different sites across Ealing may be necessary.
