We are looking for a dedicated andforward-thinkingsenior band 5 registered nurse to join our new self-care team. If you have the ability toengage staffinsupporting ourpatients on their self-care journey, you would be perfect for our team.
The successful candidatewill haveexperience of community-based care, be passionate about patient's choices & their abilities toself-manage andinstigate personalised care.
The candidate will work across the Fylde Coast, as part of a community nursing service, undertaking nursing duties in patients’ homes or clinic settings. Supporting patients towards becoming experts in their own health care interventions. To provide safe, effective, individualised care to patients and to give support to their families
Your responsibilities will be to support junior staff in delivering a high quality service, which will include oversight of clinical caseloads, clinical supervision and training. The role will also include facilitation of learning and competence for staff & patients, ensuring they feel confident and able to optimise their skills, maximising the benefits of self-management.
The candidate must be willing to undertake training around health coaching, shared decision making and the personalised care agenda, actively promoting a self-care model whilst ensuring you have the core clinical skills required.
Utilising support services throughout Fylde Coast, the candidate will support the provision of personalised high-quality care,ensurebetter patient outcomes, and positively influenceservice implementation and change.
