What you will do

• Complete Care Act assessments, reviews, and support planning.
• Undertake Mental Capacity Assessments and participate in best interest decision-making.
• Carry out risk assessments and develop comprehensive risk management plans.
• Manage a varied caseload, supporting adults with a wide range of needs.
• Investigate safeguarding concerns in accordance with statutory guidance and local procedures.
• Work collaboratively with health professionals, carers, providers, and partner agencies to deliver coordinated care.
• Maintain accurate and up-to-date case records and reports.
• Empower adults to achieve outcomes that promote independence, choice, and wellbeing.
