What you will do

  • Support adults experiencing mental health challenges through assessment, care planning, and ongoing intervention.
  • Apply your knowledge of the Care Act to develop person-centred care plans.
  • Work collaboratively with healthcare professionals, partner agencies, and colleagues to deliver coordinated support.
  • Communicate effectively with service users, families, and professionals to achieve positive outcomes.
  • Manage a community caseload while demonstrating initiative and sound professional judgement.

Qualifications/Requirements

  • Social Work England registration.
  • Recognised Social Work qualification.
  • Sound understanding of the Care Act and its practical application.
  • Experience working with adults with mental health needs.
  • Excellent communication and team-working skills with the ability to work independently.
