Key Responsibilities

  • Deliver high-quality nursing care in the community, empowering patients to maintain independence.
  • Assess, plan, implement, and evaluate person-centred care plans.
  • Collaborate with multidisciplinary teams, GP surgeries, and acute trusts.
  • Assist in facilitating smooth hospital discharges and reducing readmissions.

Clinical skill set required

  • Complex Wound care including Doppler compression
  • Simple wound care
  • End of life care
  • Medication administration
  • Catheterisation male female suprapubic
  • Bowel care
  • Syringe driver
  • Insulin administration
  • Doppler assessment
  • Compression bandaging

Essential Qualifications

  • NMC registration (valid and active).
  • A minimum of 6 months of NHS experience as a Community Nurse.
  • Eligibility to work in the UK.
