Key Responsibilities
- Deliver high-quality nursing care in the community, empowering patients to maintain independence.
- Assess, plan, implement, and evaluate person-centred care plans.
- Collaborate with multidisciplinary teams, GP surgeries, and acute trusts.
- Assist in facilitating smooth hospital discharges and reducing readmissions.
Clinical skill set required
- Complex Wound care including Doppler compression
- Simple wound care
- End of life care
- Medication administration
- Catheterisation male female suprapubic
- Bowel care
- Syringe driver
- Insulin administration
- Doppler assessment
- Compression bandaging
Essential Qualifications

- NMC registration (valid and active).
- A minimum of 6 months of NHS experience as a Community Nurse.
- Eligibility to work in the UK.
