What you will do

• Conduct assessments for individuals with learning disabilities and autism to identify needs and develop person-centred support plans.
• Provide guidance, advocacy, and practical support to help service users access appropriate services and resources.
• Work collaboratively with multidisciplinary teams, partner agencies, families, and carers to deliver coordinated care.
• Monitor and review care plans, adapting support to meet changing needs and promote positive outcomes.
• Build strong relationships with families and carers to ensure a holistic and inclusive approach to care.
• Keep up to date with current legislation, policy, and best practice to maintain high standards of professional practice.
