About this role
This role supports the 019 Team in delivering the Healthy Child Programme through a tiered approach, providing universal, targeted, and specialist services across community settings. The postholder delivers compassionate, safe, and effective care to children, young people, and families in homes, schools, clinics, and community environments. Working under the supervision of a registered nurse, they undertake health assessments, monitor child development, and contribute to early intervention programmes that promote physical, mental, and behavioural wellbeing. Key responsibilities include supporting families to improve health outcomes, delivering evidence-based interventions, and making appropriate referrals to partner agencies. The role involves strong multi-agency collaboration to ensure integrated care, safeguarding children and vulnerable individuals, and promoting resilience and life opportunities. The postholder contributes to health promotion activities, maintains accurate clinical records, and ensures care is delivered in line with professional standards and safeguarding policies. They are expected to manage their workload effectively, engage in continuous professional development, and support a positive team environment. Strong communication skills are essential, alongside the ability to build effective relationships with families, colleagues, and partner organisations, ensuring high-quality care and positive outcomes for children and young people.
