The Bexhill Irvine Unit is an intermediate care rehabilitation facility providing multidisciplinary, patient-centred care for predominantly older adults requiring ongoing rehabilitation following acute illness or hospital admission.

We are seeking a motivated and compassionate Senior House Officer (SHO/FY2 level) to join our Care of the Elderly team. This role offers an excellent opportunity to develop skills in geriatric medicine, rehabilitation, and holistic patient management within a supportive multidisciplinary team environment.

The successful candidate will provide day-to-day medical care for patients undergoing rehabilitation, working closely with consultants, specialty doctors, nursing staff, physiotherapists, occupational therapists, and social care teams. The post focuses on comprehensive geriatric assessment, optimisation of medical conditions, and supporting safe discharge planning.

This is a non-on-call post, making it particularly suitable for doctors seeking experience in elderly care, rehabilitation medicine, and multidisciplinary working in a structured and well-supported setting.

  • Clinical Responsibilities
  • Provide high-quality, day-to-day medical care for patients in an intermediate rehabilitation setting, primarily older adults with complex needs
  • Undertake comprehensive geriatric assessments, including medical, functional, cognitive, and social aspects
  • Review and manage acute and chronic medical conditions in rehabilitation patients
  • Optimise medications, including polypharmacy reviews and deprescribing where appropriate
  • Respond to clinical deterioration and liaise with acute services where escalation is required
  • Maintain accurate, timely, and clear clinical documentation

Multidisciplinary Team (MDT) Working

  • Work collaboratively within a multidisciplinary team including nurses, physiotherapists, occupational therapists, and social care colleagues
  • Actively participate in MDT meetings, ward rounds, and discharge planning discussions
  • Contribute to goal setting and rehabilitation plans for patients
  • Support safe and timely discharges, including communication with community and primary care services

Communication

  • Communicate effectively with patients, families, and carers regarding care plans and progress
  • Liaise with GPs, community teams, and acute hospitals to ensure continuity of care
  • Provide clear clinical handovers and documentation
