About this role

The Community Based Neuro Rehabilitation Team in BHFT is an established team delivering Specialist Neuro Rehabilitation for over 16 years. The team work collaboratively with other teams including Headway, the Stroke Association and other NHS and Social Care Teams.

We are very excited to be able to be recruiting for a Band 6 Physiotherapist to join the Earlier Supported Discharge (ESD) team for Stroke Rehabilitation based in the East Berkshire Locality.

As Specialist Physiotherapist in Neuro Rehabilitation, you will provide clinical delivery of Physiotherapy to patients.

This role is for 30 hours a week at Band 6 level. The role is within an MDT working closely together. The MDT consists of Physiotherapists, Occupational Therapists, Speech and Language Therapists, Clinical Neuro Psychologists, Stroke Nurses and Administrators.

Within ESD we are supporting patients to continue their rehabilitation at home from acute stroke units. Making rehabilitation at home meaningful and patient centred.

The team are an exceptional group of people who are highly engaged, innovative, forward thinking and most importantly patient centred in the rehabilitation they offer. Feedback from patient is consistently excellent and staff survey results are very strong.

The working pattern will be 30 hours a week

Work base is St Marks Hospital with rehabilitation being delivered in patients’ home and other appropriate community settings.

Key responsibilities

Clinical delivery of specialist Physiotherapy for patients needing Neurorehabilitation

To undertake comprehensive assessment of patients and formulate their treatment plan

Delegate effectively to therapy Assistants

To communicate effectively and collaboratively with other colleagues and teams within and outside of the NHS

To contribute to the development of neurological rehabilitation services within Berkshire working closely with the Team Leads for CBNRT and ESD and other members of the team

To be actively involved in the collection of appropriate data and statistics for the use of the department.

The “must haves” for this role

1.Degree-level education (or equivalent) & current professional registration

2.Experience in neurorehabilitation with the ability to support the case with complex case formulation and management of complex cases

3. Experience of working within multidisciplinary teams

4. Excellent communication, organisational and decision-making skills

5. Car driver, access to a vehicle and willingness to travel to sites where the team are based in Berkshire
