About this role
The East Suffolk and North Essex Foundation Trust is looking for a dynamic, flexible and conscientious person to join the South Rural Integrated Neighbourhood team, based at Beacon House, Ipswich, IP1 5PB
Our Rural Community covers Shotley/Holbrook, Capel and East Bergholt, Hadleigh and Bilderston GP Practices.
As a Generic worker within a community based integrated team, you will have the opportunity to support safe, effective care. You will work alongside the nursing and therapy teams with registered practitioners to support specific tasks as part of the individuals' care plan. You will have a mindset to promote independence for activities of daily living and encourage self-care wherever possible. You will carry out routine elements of assessment, work to care plans, and carry out delegated care/therapy interventions. You will also work alongside our social care practitioners, care-coordinator, community and voluntary partners utilising an MDT to achieve best outcomes for our patients.
Our supportive team will support you to develop skills, and knowledge about an integrated health and social care response to care in the community, in response to NHS plans for place-based, proactive and preventative care and digital developments.
As this role is community focused, it is essential that applicants hold a valid driver's license and daily access to a car.
- To assist in the delivery of high standards of healthcare to patients within their own homes and in clinics, by carrying out planned care.
- To provide high standard of care to the patients whilst promoting independence and respecting their privacy and that of their families.
- To undertake delegated assessments and deliver healthcare for patients.
- To work alongside the integrated team to prevent unnecessary admission to hospital.
- To work with all health care professionals, and statutory/non statutory agencies to provide a seamless, integrated service to our service users.
- Complete delegated assessments and apply competency based treatment/care/interventions to patients under the supervision of a registered practitioner, following a prescribed treatment/care plan.
- Understand own role in delivering care, enabling or providing interventions and treatments to meet individual’s needs
- Ensure digital patient records are completed for each intervention, and that Epic inputting is completed in a timely manner.
