About this role

Role Summary

The NHUC Remote Monitoring Service provides two interlinked services across North and Mid Hampshire; a proactive level 2 remote monitoring service that consists of the long term monitoring of patients with moderate/severe frailty and identifying deterioration earlier to promote community management. The service also provides a level 3 short term intensive monitoring for acute respiratory infections and exacerbations of respiratory chronic diseases. This is a rapidly expanding area of clinical care in line with the NHS England strategic direction with the shift of care into the community and supporting proactive, not reactive care. The post holder will work as part of a team of clinicians providing the service, with responsibility in particular for the following: - Level 3 onboarding and discharges - Responding to digital platform red alerts - Responding to escalated cases from other non-registered clinicians - Clinical oversight for the non-registered clinicians - Oversight of cases escalated outside of the Remote Monitoring

Team Main Duties and Responsibilities

This role encompasses the skills of leadership and clinical oversight of the Remote Monitoring Service, using telephone and video triage to carry out assessment and monitoring within an agreed scope of practice. To work collaboratively with the Clinical Head of Service Primary Care, Remote Monitoring Service Manager, Lead Nurse, and other service Clinicians and Clinical Support Workers. This may include covering each other, and in some cases working closely together and sharing a role. To provide clinical oversight and support of the non-registered Clinical Support Workers, accepting escalated cases, providing training and clinical advice to ensure patient safety at all times. To use advanced clinical skills to provide education to patients, promoting self-care, and empower them to make informed choices about their treatment. To participate in service development in conjunction with local colleagues and specialist practitioners as required To prioritise health problems and intervene appropriately, including initiation of effective emergency care via the most appropriate services. Promote health and well-being using health promotion, health education, screening, and therapeutic communication skills. To clearly communicate the health status of the patient using appropriate terminology and technology To work collaboratively with the patients own GP & multidisciplinary team responsible for their care where required To maintain accurate and contemporaneous records, utilising electronic systems and consider the Caldicott Principles in relation to all data handling. To provide professional advice to multidisciplinary colleagues in relation to specialist area of practice. To ensure adherence to the professional rules of conduct as laid down by the NMC (Nursing & Midwifery Council), HCPC (Health and Care Professions Council) or equivalent professional body. The Remote Monitoring Service will be run by North Hampshire Urgent Care working alongside other key external stakeholders. Governance Structures will be determined by North Hampshire Urgent Care. Administrative and professional responsibilities

Oversight of the remote monitoring platform, to ensure patients are not missed from the appropriate level of escalation/follow up. Participation in team meetings to discuss patients on the Remote Monitoring service as required Ensure accurate notes of all consultations and treatments are recorded in the patients notes on the clinical computer systems as appropriate. Ensure accurate completion of all necessary documentation associated with patient health care including EMIS Web. Keeping up to date with service information sent by email.

Liaison

Maintain effective liaison with other agencies and staff concerned with patient care and with all other disciplines with appropriate regard to confidentiality.
