Multiple Sclerosis Care Co-Ordinator

The MS Care Coordinator provides comprehensive coordination and administrative support for people living with Multiple Sclerosis (MS), acting as a key point of contact for patients, carers and healthcare professionals. The post holder coordinates patient pathways, supports Disease Modifying Therapy (DMT) monitoring programmes, and ensures timely access to reviews, investigations and specialist services. Working closely with the multidisciplinary team, they facilitate communication across acute, community and primary care settings, maintain accurate patient records, and support service improvement initiatives. The role promotes a high-quality patient experience through effective coordination, compassionate communication and proactive management of care pathways.
