About this role
The Care Coordinator will work as a key part of the Primary Care Network (PCN) multi-disciplinary (MDT) team. You will work closely with the GPs and other primary care professionals within the PCN to identify and manage a caseload of identified patients, making sure that appropriate support is made available to them and their carers, and ensuring that their changing needs are addressed. The role contributes to tackling inequalities in health and social care. An ethos of promotion of independence, shared decision making, personalisation and partnership working is integral to this post. As the role evolves the care coordinator may undertake direct work with patients and families to develop personalised care plans Key Responsibilities Provide a first point of contact for patients and clinicians in coordinating patients care. Deal with incoming queries from patients and-or their carers and other healthcare providers. Encourage people, their families and carers to provide feedback and engage fully in the care coordination process. Ensure timely follow up and action for patients from communications from community and secondary care. Provide coordination and navigation for people and their carers across health and care services, working closely with social prescribing link workers and other primary care professionals. Signpost and organise appointments, follow ups or other actions to help the PCN provide high quality, compassionate care to our patient population. Support the PCN in coordinating all key activities including access to services, advice and information, and ensuring health and care planning is timely, efficient and patient-centred. Work collaboratively with other Care Coordinators across the PCN to share best practice. Work sensitively with patients, their families and carers to capture key information, enabling comprehensive and accurate records. Referral enquiries are passed on to the referrals admin team in a timely manner to deal with. Request prescriptions received via the online form are given to the named GP or duty doctor Request Fit notes with the named GP or duty doctor if urgent Process appointments in response to patients recalled for Long Term Condition reviews, cervical screening, immunisations etc Arrange appointments for blood tests due for medication reviews, and liaise with the named GP for any more complex patients. Coordination of women's health appointments for Coils and implants with appropriate GPs within the practice. Applicable to all staff CONFIDENTIALITY In the course of your employment, you will have access to confidential information relating to the PCN and GP Practices. You are required to exercise due consideration in the way they use such information and should not act in any way that might be prejudicial to the PCNs interests. Information which may be included in this category which requires extra consideration covers information relating to the general business of the PCN and information regarding individuals. If the post holder is in any doubt regarding the use of information in the pursuit of their duties, they should seek advice from the PCN Manager before communicating such information to any third party. In the course of seeking treatment, patients entrust the company with, or allow us to gather, sensitive information in relation to their health and other matters. They do so in confidence and have the right to expect that staff will respect their privacy and act appropriately. This information is to be treated as strictly confidential. Information relating to patients, carers, colleagues, other healthcare workers or the business of the practice may only be divulged to authorised persons in accordance with the company policies and procedures relating to confidentiality and the protection of personal and sensitive data. DATA PROTECTION and GDPR S&C PCNs Ltd, must be compliant with the Data Protection Act 1998 and GDPR 2018. You must not at any time use the personal data held by the PCN or any of the GP Practices or disclose such data to a third party for a purpose not described in the Register entry. If the post holder is in any doubt regarding what they should or should not do in connection with the Data Protection Act 1998 or GDPR 2018 then they must contact the PCN Manager. FINANCIAL REGULATIONS All employees are responsible for the security of the property of the PCN avoiding loss of or damage to property and being economical and efficient in the use of resources. SAFETY Employees must be aware of the responsibility placed on them under the Health and Safety at Work Act 1974 to maintain a healthy and safe working environment for both staff and visitors. Employees also have a duty to observe obligations under the PCNs Health and Safety policies and to maintain awareness of safe practices. The jobholder is required to take reasonable care for the safety and health of themselves and others who may be affected by their acts, and to co-ordinate with management in the promotion and maintenance of health and safety measures. The post holder will assist in promoting and maintaining their own and others health, safety and security as defined in the practice Health & Safety Policy, to include: Using personal security systems within the workplace according to practice guidelines Identifying the risks involved in work activities and undertaking such activities in a way that manages those risks Making effective use of training to update knowledge and skills Using appropriate infection control procedures, maintaining work areas in a tidy and safe way and free from hazards Reporting potential risks identified. This job description is designed to reflect duties currently incorporated in this post. These may change in light of a change in the service provided by the PCN, but any such change will be fully discussed with the post holder. This job description is subject to an annual review.
