About this role

Job Summary The Practice Nurse will work as part of the primary healthcare team providing Personal Medical Services to the patients of Practice and, working in accordance with the NMC The Code, the post-holder will provide assessment, treatment, screening, health education services and advice to patients. The post holder will provide and maintain a high standard of nursing care for patients as well as providing nursing assistance to the doctors and other members of the primary healthcare team. The duties will include all tasks normally undertaken by an experienced RGN and in addition, any roles agreed between the nurse and the doctors as appropriate, having regard to current training and competencies. The post-holder will work with reference to Practice agreed protocols and guidelines, providing nursing treatments to patients both independently and in participation with nurse colleagues and general practitioners. The post-holder will have the required level of training and competence and will work within patient group directions where these are available. A nurse with prescribing qualifications will use them appropriately.

Plan, develop and implement approaches to promote self-care and independence and prevent adverse effects to patient health, independence and well-being. Work with patients and carers to support and promote individuals rights and responsibilities. Plan, deliver and evaluate interventions and treatments for chronic disease management. Demonstrate a high level of complex decision making in relation to the assessment, planning, intervention and evaluation of patient care. Establish effective working relationships with patients, their families and carers. Recognizing and respecting their contributions to care planning. Promote, monitor and maintain best practice in health, safety and security. Establish and sustain effective working relationships across all health and social care service organizations. Provide information and advice at all stages of the patients pathway, including the management of emergencies and exacerbations. Document all interactions with patients, carers and professionals clearly, concisely and in a timely manner. Key Tasks A Assist in and perform routine tasks related to patient care as directed by GPs Cytology and taking swabs. Travel Clinic / Advice ECGs. Asthma and COPD reviews Venepuncture (Adults and suitable under 18s) and urinalysis. Adult and Childhood immunisations including maintenance of the cold chain policy Chaperoning and assisting patients where appropriate who are being examined by another clinician Assisting GPs with minor surgery and coil fittings Requesting pathology tests, for example urine culture, swabs Chronic disease management including Diabetes, Hypertension and Heart Failure Contraceptive advice including administration of injectable contraception Physical Mental Health Reviews Learning Disability reviews Health Education, including Non-Diabetes Hyperglycaemia and Raised Cholesterol Cardiovascular risk assessment and advice Weight management advice NHS Health checks and new patient review. Sexual Health Advice Alcohol Screening and Smoking Cessation advice Following agreed clinical protocols with referral to GPs as appropriate Diabetic Foot Screening Infection Control Prevention
