Administrative Support Provide full administrative support to the Trauma-Informed Care Lead and MDT Coordinate and support monthly MDT case review meetings and case conferences Prepare agendas, take minutes, and manage action trackers Arrange clinics, meetings, and appointments Maintain accurate records and filing systems (electronic and paper-based) Act as a point of contact for people with high levels of service use, their carers, and partner organisations, ensuring clear communication, signposting, and trauma-informed coordination Ensure administrative processes reflect trauma-informed principles, including sensitivity in communication and awareness of patient triggers Data Management Maintain and update databases and tracking systems for people with high levels of service use Ensure data accuracy, completeness, and quality Input, extract, and manage data from clinical and non-clinical systems Support data linkage across services where appropriate Collect and monitor activity data including admissions, contacts, outcomes, and patient-level metrics Reporting & Evaluation Produce routine performance reports (monthly/quarterly) for internal and external stakeholders Support monitoring of key performance indicators, including: A&E attendances Hospital admissions Service utilisation Engagement with programme interventions Contribute to evaluation of programme outcomes and benefits Assist with ROI and SROI reporting processes Prepare data summaries, dashboards, and presentations Produce reports relating to people with high levels of service use for MDT partners, ICS colleagues, and voluntary-sector organisations MDT & Programme Coordination Act as a key point of contact for programme administration Support coordination of case reviews and care planning activity for people with high levels of service use Liaise with MDT partners (GPs, community teams, mental health, VCSE) Support information sharing in line with governance requirements Identify gaps or delays in workflows and escalate appropriately Coordinate referrals, updates, and feedback loops with voluntary-sector partners delivering interventions
