About this role
We are looking for a compassionate and proactive Social Prescribing Link Worker to support individuals, 18+ living in Milton Keynes, with non-medical needs by connecting them to local services, groups, and activities.
What you’ll do
- Provide one-to-one, person-centred support
- Develop personalised wellbeing plans
- Link people to community resources
- Work with GP practices and local partners
What we’re looking for
- Experience in health, social care, or community work
- Strong communication and organisational skills
- A genuine interest in green spaces and nature-based activities
- Empathy and a non-judgemental approach
- Driving Licence and car essential
