Diabetes Specialist Nurse

About this role

To demonstrate a professional approach to the leadership and management of the in-patient service. Assumes accountability and responsibility for practice and along with the members of the healthcare team, strives to attain the highest standard of practices. To be recognised as the clinical expert for diabetes representing the acute trust to external agencies. To be able to disseminate appropriate information to others across the acute trust. Nurse/Patient relationships Demonstrates and establishes a relationship of trust with patients, carers, and families. Demonstrates expertise when dealing with complex patients with co morbid conditions and their families who require new skills to live with a chronic condition. Communicates and agrees with the patient, carer, and family the determined plan of care and to ensure the patients interests are fully considered within the context of care actively partaking in ethical decision making which can be complex and sensitive. Management of patient ill health/illness status Receives direct referrals from medical staff, nursing staff and patient self-referral. Is able to review appropriateness of these referrals. To act as an expert practitioner in the management of the patients chronic condition, demonstrating advanced clinical skills and knowledge to professional staff and patients with a diabetes condition. Demonstrate awareness of the management of contentious information from patients in relation to their condition and treatment, which may include refusal of further interventions / treatment. Education Identifies the ongoing training and educational needs of the person with diabetes, as well as their carers and their families in relation to the treatment and management of different types of Diabetes and the prevention of related complications. Provides specialist, expert education to the patient and their families as well as to other health professionals and stakeholders. Please refer to the job description for further details.
