Are you an experienced Healthcare Assistant or someone with experience supporting adults in health or social care, looking to develop your skills within a community-based therapy role?

An excellent opportunity has arisen for a Band 4 Multi‑Therapy Assistantto join a friendly, supportive, and well-established integrated community team within Berkshire Healthcare NHS Foundation Trust (BHFT).This is an exciting time to join the service as we continue to develop and strengthen integrated ways of working across West Berkshire and Berkshire West.

The successful candidate will work alongside a knowledgeable and experienced multi‑disciplinary team, including Senior Specialist Community Physiotherapists, Occupational Therapists, Therapy Assistants, and administrative colleagues. You will be an integral member of the Clinical Support Team, contributing to the delivery of high-quality, responsive, and person‑centred care in the community.

Our integrated service supports a range of pathways including Community Reablement, Falls Prevention, and Community Physiotherapy, working closely with health, social care, and voluntary sector partners. This role involves community visits, offering variety and autonomy, with agile working opportunities to support work–life balance, and development will be supported through supervision and training.

You will support adults with a wide range of physical health conditions, functional impairments, and mental health needs, helping people to maximise independence and remain safely at home.

• Work as part of themulti‑disciplinary team to support the assessment, treatment, and rehabilitation of patients across all service pathways.
• Contribute to discussions regarding patient progress and outcomes, providing feedback to the wider team and participating in reviews as appropriate.
• Carry out therapy and exercise programmes as advised by registered clinicians, supporting patients to improve mobility, function, and activities of daily living (ADLs).
• Maintain a patient‑centred approach, promoting dignity, independence, and choice in all interactions.
• Liaise effectively with Registered Nurses, Occupational Therapists, Physiotherapists, Care Managers, and Social Workers, ensuring accurate, timely, and concise documentation.
• Participate in team meetings, supervision, and professional development activities, demonstrating a commitment to learning and service improvement.
• Comply with Trust policies, safeguarding responsibilities, and information governance requirements at all times.

The “must haves” for this role

• NVQ 3 or Equivalent experience.
• Experience of working with older people .
• Experience of working in health/social care at Band 3 level or equivalent
• Excellent communication skills
• Ability to Drive & have use of a car
