About this role

An exciting opportunity has arisen for a Band 7 annex 21 Stoma Specialist Nurse. The successful applicant will commence on a annex 21 development pathway, where by once competencies are completed will progress to Band 7.

The key purpose of the post is to undertake a range of expert clinical nurse specialist tasks to support and care for all patients in the East Sussex area with a stoma, in both the acute and community settings.

To complete home visits, working autonomously to support patients with stomas.

Manage and facilitate adhoc and pre planned nurse led clinics to support and guide all patients with stomas.

Work within the wider MDT to help facilitate and support staff and patients alike in an expert manner to ensure all patients with a stoma receive an excellent high level of care.

Acting as a clinical expert, the post holder will facilitate the delivery of high practice standards and promote the integration of research into care. Recognising this expertise, the post holder will work and support junior members of the MDT in both the acute and community settings.

Assist in planning and delivering strategic stoma care services aligned with Trust priorities, including policy development and liaison with ostomy manufacturers. Support supply management, helpline operations, patient support groups, newsletters, and events. Maintain a personal caseload, providing pre- and post-operative education, stoma site marking, discharge planning, and follow-up care. Deliver impartial advice on appliances and accessories, manage complex cases, and ensure accurate documentation.

Act as a role model, working autonomously while maintaining up-to-date knowledge in stoma and pouch care. Uphold professional standards, ensure effective MDT communication, and contribute to service quality through audits, research, and product evaluations. Support education and training across acute and community settings, including university and residential care sessions. Identify patient/carer learning needs and mentor staff. Promote a safe, confidential, and positive environment, attending relevant study days and pursuing further qualifications.
