Key Responsibilities Key
Responsibilities Clinical Assessment & Care
Perform home visits, assess undifferentiated patients and treat minor illnesses /injuries.
Patient Management
Diagnose acute and chronic conditions, prescribe medications and manage patient care plans
Urgent Care Response
Provide rapid, expert care to patients, including telephone triage and home based interventions to prevent hospital admission.
Primary Care Support
Work within GP practices or Primary Care Networks (PCNs) to manage complex caseloads.
Health Promotion
Advise patients on health maintenance self-care and medication management.
