Nursing Associate

About this role

The Nursing Associate plays a vital role within the Urgent Community Response (Home 1st) service, delivering highquality, compassionate, patientcentred care to people in their own homes. The role supports timely assessment, intervention and shortterm care to promote independence, prevent unnecessary hospital admissions and identify deteriorating patients early. Clinical Care Deliver highquality, personcentred care to patients in their own homes, promoting independence, health and wellbeing and helping to prevent unnecessary hospital admissions. Undertake a wide range of delegated clinical tasks within scope of practice, including clinical observations (using NEWS2), wound care, pressure ulcer prevention, catheter care (including suprapubic), venepuncture, ECGs and blood glucose monitoring. Prepare and administer medicines within the limitations of the Nursing Associate role, in line with Trust and national policies. Urgent and Community Care Recognise, respond to and escalate signs of patient deterioration promptly, seeking advice and guidance from Registered Nurses or other registered professionals as required. Contribute to rapid assessment and shortterm intervention as part of the Urgent Community Response (Home 1st) service, supporting safe and timely care at home. Risk Management and Patient Safety Assist in the identification, assessment and management of clinical risk, including falls, nutrition, moving and handling and pressure damage, in line with Trust policies. Maintain patient privacy, dignity and safety at all times. Report incidents, risks and concerns using DATIX and escalate issues appropriately. Communication and Team Working Communicate effectively with patients, carers, Registered Nurses and the wider multidisciplinary team, sharing relevant information to support coordinated care. Work collaboratively with partner services including health, social care and voluntary organisations within scope of practice. Professional Practice Practise in accordance with the NMC Code, remaining accountable for own actions and working within defined limits of competence. Maintain accurate, timely and professional clinical records using electronic systems in line with Trust policy and NMC standards. Take responsibility for own learning, reflective practice and continued professional development, meeting NMC revalidation requirements. Support and contribute to the learning and development of students, trainees and support staff as required. Governance and Service Improvement Participate in audit, service improvement and quality activities within the service. Use equipment and resources safely and economically, reporting faults, hazards or equipment failure. Comply with Trust policies relating to safeguarding, infection prevention and control, confidentiality, information governance, equality and diversity, and health and safety.
