About this role
As a Senior Clinical Assessor/Practitioner (Registered Mental Health Nurse, Registered Learning Disabilities Nurse or SWE registration with mental health specialism) in Specialist s117 Aftercare, you will be responsible for supporting clients who require care, treatment and support under the Mental Health Act, conducting comprehensive individual assessments of clients, case managing clients to ensure that their needs are effectively met and promoting Personal Health Budgets and personalisation to empower people with mental health needs to live their best lives. You will need to have excellent interpersonal, presentation and communication skills to establish good rapport with people with Mental Health, Autism or both and an ability to make decisions in a challenging but supportive environment. As part of the role, you will have the opportunity to develop strong links with health and social care colleagues to ensure effective multi-disciplinary working and develop a strong understanding of local NHS and independent sector service provision in individual's own homes and in care homes. The successful candidate will be able to use specialist knowledge and clinical skills to ensure that coordination and outcomes are determined in a consistent manner to ensure effective application and interpretation of the MHA S117. Whilst experience in the Mental Health Act would be helpful, if you have the right skills and attitude, we can support you to develop the specific skills required. The Trust has a suite of educational opportunities to develop skills and we offer team training opportunities and Team Leaders who are non-case holding in order to mentor and develop our Clinical Assessors with complex cases, including safeguarding, Mental Capacity Act and Court of Protection. You will be part of a wider All Age team, comprising administration, brokerage, commissioners, Deprivation of Liberty experts, proportionate care practitioners and case practitioners who provide a wide range of expertise and support.
